What operating system are you using? Some people have had problems with Windows XP, you may need to run Win32DiskImager as admin right click — run as admin. Others have simply had issues where the card reader seems to be incompatible and the only solution has been to try a different one.

NOOBS was specifically created for that purpose — to make it easy for people to do that. I generally flash my SD cards with the Raspbian image using dd in linux now.
